About the Position: 
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities
What to Expect:
<ul>
<li>Conduct Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up</li>
<li>Provide services in a variety of venues to include: Home Visits, Skilled Facility Visits</li>

Type: PRN - Flexible - Nurse Practitioner
PAY: Earn from $2,100 up to $10,500 per month (based on an average of 20-100 completed visits per month and pay of $105.00 per completed assessment). Compensation range is based on availability and assessment completion.
Location: Home, SNF, and other community environments in Boston, Massachusetts
Hours: Days, Evenings, Nights, Weekends, Flexible Scheduling

Qualifications
What is Required:
<ul>
<li>Master’s Degree OR commensurate experience and satisfactory completion of NP licensure</li>
<li>Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority</li>
<li>Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty</li>
<li>Current BLS, ACLS or CPR certification</li>
<li>Minimum of 1 year experience as a Nurse Practitioner</li>
<li>Strong computer skills and familiarity with employee health/medical record software</li>
<li>Excellent verbal and written communication skills with patient, clients, and colleagues</li>

The Registered Nurse (RN) in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.&nbsp;&nbsp;
<ul>
<li>Coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.&nbsp;</li>
<li>Makes the initial and/or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within state specific guidelines, and submits accurate, complete, and timely documentation in collaboration with physician orders.&nbsp;&nbsp;</li>
<li>Performs patient comprehensive assessments at designated time points and develops the appropriate plan of care.&nbsp;</li>
<li>Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.</li>

License Requirements

️‍⚕️ Your Role as a Registered Nurse:
You'll be the clinical cornerstone for our patients, providing expert care and leadership in the comfort of their homes. As an RN, you'll assess, plan, and manage patient care while collaborating with a multidisciplinary team that shares your commitment to quality.
 Key Responsibilities:
*Conduct comprehensive initial and ongoing assessments based on individual patient needs and provider orders
*Develop and continually update care plans to reflect evolving conditions and goals
*Deliver skilled nursing interventions within the scope of practice and in accordance with physician directives
*Maintain detailed and compliant documentation per all state, federal, and agency regulations
*Lead case conferences, promoting collaboration between caregivers and clinicians

✅ What You'll Need:
*Graduate of an accredited Nursing Program (RN)
* Active Registered Nurse license in MA
* Valid CPR/BLS certification
* Reliable transportation &amp; valid driver's license
* 1+ year of experience in acute care, home health, outpatient, or community health (within the last 2 years)
* Up-to-date health certificate &amp; TB test
*Clear understanding of HIPAA and strong commitment to confidentiality
* Fluent in English (spoken &amp; written)
*Must be at least 18 years old and eligible to work in the U.S.
 Compensation:
