Major Functional Areas and Description of Duties

<li>Engage clients and visitors during check in and check out process.</li>
<li>Collect co-payments, outstanding balances, authorizations and signatures on needed forms ensuring all receivables are appropriately collected and documented.</li>
<li>Scan in and appropriately label paperwork into electronic medical record.</li>
<li>Answer phones professionally and direct calls to appropriate staff.</li>
<li>Provide administrative support via centralized scheduling practices by setting up appointments immediately following a session and/or outreach after a missed appointment occurs.</li>
<li>Coordinate staff calendars, set up video conference meetings, and assign treatment rooms as needed.</li>
<li>Stock office supplies, equipment, and other materials to be readily available for use by all.</li>
<li>Promptly communicate concerns, questions, and/or important information with appropriate Manager, clinical supervisor, administrative team member, clinical and/or medical staff.</li>

<li>Support the efforts of each department in ensuring a wrap-around systems approach is provided to the client.</li>

As a Medical Case Manager you will make a meaningful difference in the lives of injured workers and their families. Your responsibilities include working closely with injured workers to facilitate their recovery. You will work collaboratively with the patient, their family, medical providers, members of our team, and others. Pay rates are established taking into account the following factors: federal, state, and local minimum wage requirements, the geographic location differential, job-related skills, experience, qualifications, internal employee equity, and market conditions. Our ranges may be modified at any time.
For leveled roles (I, II, III, Senior, Lead, etc.) new hires may be slotted into a different level, either up or down, based on assessment during interview process taking into consideration experience, qualifications, and overall fit for the role. The level may impact the salary range and these adjustments would be clarified during the offer process.

BAYADA Home Health Care is currently seeking an energetic and experienced Medical Social Worker, MSW, for a per diem opportunity performing home health visits our Londonderry Office. This office services clients on a per visit basis in territories throughout New Hampshire
Prior home care experience preferred, but not required. As a Medical Social Worker, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home. 

Responsibilities include:

<li>Making home visits to clients in designated geographic territories.</li>
<li>Assist our team in understanding the social and emotional factors related to our clients’ health problems.</li>
<li>Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.</li>
<li>Identify appropriate community resource referrals to address the client’s and the family’s practical and environmental needs.</li>
<li>Act as an advocate for both the client and the family to navigate the patient through the community system.</li>
<li>Participate in the development of the total plan of care with the multidisciplinary team of BAYADA clinicians, as appropriate.</li>
<li>Develop and maintain contracts with public and private agencies as resources for the patient.</li>
<li>Educate and instruct patients, family members, or other patient representatives in community resources to promote the patient's health, safety, well-being, and independent living.</li>
<li>Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided utilizing a state-of-the-art touch pad tablet.</li>

Qualifications for the ideal candidate:

<li>A minimum of a Masters in Social Work, with at least 2 years of community-based experience.</li>
<li>Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree.</li>
<li>Home care experience a plus.</li>
<li>Demonstrated ability to read, write, and effectively communicate in English.</li>
<li>Ability to work independently and manage time effectively.</li>
<li>Strong interpersonal skills.</li>
<li>Solid computer skills; prior experience with electronic medical records (EMR) preferred.</li>
<li>Ability to travel to cases, as assigned.</li>
