Minimum Requirements:
1. Degree in Clinical Social Work or Master’s degree in counseling or psychology from accredited school. LMHC or LMFT are also acceptable educational credentials.
2. Knowledge in brief, integrated, trauma informed skills.
3. Language skills in Spanish, Russian, Arabic, or other language spoken by CHC pts is preferred but not required.
4. Familiarity with multicultural groups, previous work experience in a multicultural setting.
5. Strong interpersonal, written, and oral communication skills; proven ability to communicate with diverse audiences.
6. Ability to demonstrate professionalism and respect; regard for patient privacy is essential.
7. Organizational skills a must.
8. Ability to work independently and with a team.
9. Computer literacy.
10. Experience with an Electronic Health Record is preferable.
Essential Responsibilities and Duties:
1. Collaborate with the Pediatrics team to identify, offer outreach and deliver care for those patients in need of psychosocial services.
2. Provide resources and behavioral health / social interventions as needed to restore and preserve the integrity of the child’s family.
3. Provide links for adolescents to community resources in the areas of employment, education, housing, substance abuse services, etc.
4. Maintain a caseload of clients in need of counseling and case management.

  1. Provide clinical supervision and support to Pediatric Care Coordinators, Community Health Workers, and other support staff.
  2. Maintain and complete in a timely manner all appropriate paperwork and documentation in compliance with department policy and procedures.
  3. Develop and facilitate psycho-educational or social support group related to the pediatric services and /or family system.
  4. Develop and build upon existing collaborative relationship with external agencies related to child and family social services needs.
  5. Work collaboratively within interdisciplinary team and between all departments.
  6. Provide therapeutic interventions including adult and child individual, family, couple, consultation, collateral interventions (e.g., Parent Management Training model).
  7. Participate in ongoing continuing education and training activities as needed.
  8. Other duties as assigned to support the mission of the organization. For example, active participation in pediatric and adolescent health-related grant programs, quality improvement projects, and teams.

Working Conditions:

This position requires the ability to use a computer workstation.
