Full-Time School-Based Case Manager Position in Rochester, New Hampshire!

Responsibilities:

• Coordinate services for students with special needs and create individualized plans.

• Collaborate with teachers, parents, and specialists to implement IEPs.

• Monitor student progress and adjust services to meet changing needs.

• Provide guidance and resources to families to support student success.

Qualifications:

• Active State Licensure or Certification in New Hampshire.

• Previous school-based experience in case management or a related field.

• Bachelors or Masters degree in Social Work, Psychology, or a related field.

Position Details:

• Classroom: K-12

• Hours per Week: Full-Time (35-40 hours)

• Weekly Pay: Competitive pay based on experience

Contract Duration:

• Current school year with the option to extend for summer school and the 2024-2025 school year.
