Position Summary: Work with the Director of the Long Term Care Ombudsman Program to manage the responsibilities of the program and to enhance the quality of life of nursing and rest home residents by advocating for the residents of assigned facilities. Serving as a link between residents, families, staff, and others to resolve resident complaints and concerns.

Essential Functions:

  1. Advocate for the residents in the nursing and rest homes within in the catchment area of the LTCOP of AgeSpan, Inc. (which also includes Mystic Valley, Chelsea, Revere and Winthrop).
  2. To assess, resolve and provide information and referral for concerns of residents, families and the community that are received during a facility visit.
  3. To complete weekly visits to assigned facilities (Approximately 15 facilities in the towns of Chelmsford, Chelsea, Everett, Revere and Westford), unless otherwise authorized by the Director.
  4. Maintain open communication during visits with assigned facility contact person to resolve concerns of residents brought forward during visits.
  5. Complete visit reporting requirements within 48 hours of completion of a visit, in accordance with the reporting policies for visits and case documentation.
  6. Identify and report issues that may affect the health, safety, welfare, and resident rights and or any serious concern, including reports or observations of abuse, neglect, or mistreatment immediately to the Director.
  7. Maintain confidential information regarding the residents, their concerns only to be shared within the Ombudsman Program and with consent of the resident to all others.
  8. At minimum 20 % of this position is required to be on premises in the Danvers site to assist with phone intakes and consultations, unless otherwise authorized by the Director.
  9. Notify the Director immediately of any conflicts of interest that may arise during time as serving as an Ombudsman.
  10. To be knowledgeable about federal and state regulations that affects the quality of life of residents.
  11. Consult with Facilities as requested to address concerns and needs of residents.
  12. Maintain case records and visitation reporting and perform other administrative functions as needed.
  13. Provide coverage for Facilities when a Volunteer Ombudsman is not available.

Additional Functions/Requirements:

  1. Successfully complete a certification training presented by the Massachusetts Office of the State Long Term Care Ombudsman.
  2. Attend the all LTCOP Staff Meetings and Agency All Staff Meeting as well as other agency related trainings and activities.
  3. Complete re-certification training every two years, regardless of when first certified.
  4. Complete 24 hours of continuing education per year.
  5. Participate in Field Supervision with the Director or their Designee as required.
  6. Attend mandatory agency and state meetings as well as other agency related trainings and activities.
  7. Complete additional tasks and duties as assigned by Supervisor.
  8. Bi-Lingual English/Spanish Preferred but not required.

Qualifications: At least one to three years of prior work experience with elders in community or long term care settings and/or with diverse, disadvantaged adults or currently pursuing or have already obtained a degree in human, health, advocacy, or legal services. Excellent communication, computer skills and highly organized with the ability to be flexible. Can establish and maintain ongoing relationships with both individuals and organizations.

Currently, certified as an Ombudsman or obtain certification within 60 days of employment.


Hours per week: Part-time: 20 hours per week
