Geriatric Support Services Coordinator

About the Role
The Geriatric Support Services Coordinator works as part of the Senior Care Options Primary Care Team to connect members with resources that improve daily living, independence, and overall quality of life. You’ll coordinate services, problem-solve, and help individuals access the support they need to thrive.

Schedule: Monday–Friday, 9–5 (34 hours/week, includes one short day & an unpaid lunch hour daily)

What We’re Looking For

  • Strong communication and teamwork skills—able to collaborate, take initiative, and adapt to challenges
  • Proficiency with computers and documentation, with the ability to work independently and stay organized
  • Bachelor’s degree in social work or a related human services field (required)
  • Comfort working with older adults and local community resources
  • Valid MA driver’s license and reliable car (regular local travel required)
  • Ability to sit at a desk and use a computer for extended periods, and occasionally lift up to 15 pounds
