Job DescriptionNorth Suffolk is looking for a Nurse Care Manager to join our Behavioral Health Community Partner (BH CP) team! The team is fun, flexible, and diverse, working together toward a common goal and sharing our strengths and perspectives. The Nurse Care Manager serves as a member of the BH CP team, providing direct care management and coordination to Enrollees. The Nurse Care Manager collaborates with medical and other care providers and provides consultation and guidance to team members regarding health needs, health behaviors and medications of the team’s Enrollees; as well as participates in the ongoing development of trainings for staff and Enrollees regarding health and health behaviors. This position is 20 hours per week; Monday-Friday during daytime hours doing work in the office, some in the community seeing members, and some remote work flexibility. What you’ll do:
