Job Duties/Responsibilities
Direct Care 
<ol>
<li>Help individuals identify and develop interests and hobbies. Facilitate community relationships and friendships which align with individuals’ interests.</li>
<li>Empower individuals to advocate, make decisions, and become more independent in their homes and communities. </li>
<li>Assist individuals to complete personal care tasks, which include, but are not limited to toileting, showering/bathing, grooming, personal hygiene, assisting with mobility, repositioning, dressing, and range of motion exercises. </li>
<li>Support individuals in the completion of Activities of Daily Living (ADL) tasks, which include but are not limited to cooking, cleaning, personal hygiene, shopping, and money management. </li>
<li>Use supportive and adaptive, health-related protections/equipment as prescribed. </li>
<li>Assist in the development and implementation of Individual Service Plans (ISP’s) including implementation of behavioral plans and medication treatment plans. </li>
<li>Provide transportation for individuals to and from day programs, medical appointments, and other activities as required. </li>
<li>Follow the Department of Public Health’s (DPH) Medication Administration Program (MAP) policies (when applicable), which include, but are not limited to training requirements, medication administration, medication security, and corresponding documentation. </li>
</ol>

<li>Utilize Positive Behavior Supports (PBS) practices to create a thoughtful and supportive environment by ensuring activities, plans, and routines reflect the preferences, needs, and goals of individuals. </li>
<li>Use integrated strategies to support personal strengths and assets of each individual served.</li>
<li>Support individuals to make decisions across all environments to the greatest level of each person’s ability. </li>
<li>Role model behavioral awareness through demonstrating and encouraging pro-social behaviors and maintaining appropriate professional boundaries with individuals served. </li>
<li>Learn and utilize communication tools which include but are not limited to email, tablets, and letter boards, taking into consideration the method most suited to individual needs. Communicate in the preferred method and language of those you provide support.</li>
<li>Promote healthy living and relationship building, supported through individualized social, spiritual, and recreational opportunities offered in membership, access, and integration in their communities. </li>
<li>Assist with access to community, including transportation as needed, to individuals served to access community. Examples include but are not limited to transporting individuals served to and from day programs, work, medical appointments, to family and friends visits, and other recreation and leisure activities as assigned. </li>

Communication, Documentation &amp; Teamwork 
<ol>
<li>Contribute to creating an effective team environment by being solution oriented, communicating with others in a professional and respectful manner, and fostering a happy and strength-based culture.</li>
<li>Document data throughout shifts, including but not limited to medication administration, community-based and skill building activities, and goal attainment.</li>
<li>Ensure client safety and compliance with program standards for staff coverage, including assistance to the administrator on call whenever possible. </li>
<li>Identify and communicate problems, concerns, or challenges to supervisors and follow funder reporting requirements. </li>
<li>Demonstrate commitment to all individuals served and follow supervisors’ request to work in other nearby programs as needed and assist with staff coverage.</li>
<li>Float to other programs to provide support when needed. </li>
<li>Attend staff meetings, complete all required trainings, and follow all funder, state/federal, and BAMSI policy. </li>

Education/Licensure Requirements
<ul>
<li>High school diploma or equivalency. </li>
<li>Certified Nursing Assistant, Associate’s degree, or Bachelor’s degree preferred. </li>
<li>Must possess a valid driver’s license in state of residence, a satisfactory driving record, and at least 3 years of driving experience. Ability to show proof of valid driver’s license upon hire and thereafter as requested. Employees must follow the standards set by BAMSI for good driving as outlined in the Safe Driver policy and must remain in compliance with the safe driving training.</li>
<li>In instances where employees are asked to use their own vehicle to drive/transport individuals served, the appropriate automobile insurance coverage on their vehicle will be required to be shown upon hire and annually maintained thereafter. In the event of an accident while operating the employee’s own vehicle, the employee’s automobile insurance would remain the primary insurance and BAMSI insurance would be secondary. Employee vehicles must also be in good working order and have valid documentation, including a valid inspection sticker and registration. </li>
