Assist residents in dressing and personal hygiene. Assist CNA’s with residents requiring 2-staff transfer except in mechanical transfers. Make occupied bed. Assist in obtaining weights. Provide full or partial bed bath. Perform incontinent care in bed to residents who can move in bed. Assist residents who are not on swallowing or aspiration risk with meals. Pass, set up, and collect trays. Place belongings within resident’s reach (call light, glasses, TV remote, glasses, etc.) Answers call bells in a timely manner. Make unoccupied bed. Empty commodes and urinals. Transport residents to the dining room for meals. Accompanying residents on outside appointments as needed. Visit with residents providing 1:1 supportive interaction (i.e., read their mail, newspapers, or books to them, transport them for visits outdoors when within residents’ care plan, transport to activities, transport to hairdresser, help to write letters, make telephone calls, etc.) Assist residents in communication involving technology (zoom, face time, skype, google meet, etc.). Assist with monitoring residents to maintain safety providing 1:1 supervision as needed. Organize, straighten, and clean closets and dressers (i.e., clothes hung neatly on hangers in closet, folded neatly in drawers, trash and soiled laundry removed.) Ensure personal care items are put away and stocked (check with Team Leaders or Nurse Manager for appropriate stock.) Gather supplies need for provisions of care, passing out linens. Pass out drinks to independent residents who are not at risk for impaired swallowing/aspiration, check residents' drinks at bedside and verbally encourage residents to drink. Report resident’s intake to Team Leaders. Offer the resident nourishments and/or distribute nourishments from the dietary department. Check fall precautions: bed monitor, mats, and personal alarms. Check and clean dirty wheelchairs after each meal or after incontinent episode. Check and clean tub and shower room. Greet newly admitted residents upon admission, escort to room as necessary, receive paperwork from referring agency, and inform Unit Manager/Charge Nurse of arrival. Assist with inventory of new admissions. Assist in arranging for and making appointments for diagnostic therapeutic services. Direct visitors to residents' rooms, office areas, etc. Photocopy and fax per the needs of the unit.

The Female Behavioral Home Direct Support Professional responsibility is to be a community connector and will be responsible to do community mapping with the individual to promote social inclusion. The Female Behavioral Home Direct Support Professional will explore new opportunities for individuals to whom they provide supports and will assist with growth and unity within the community. Promoting abilities not disabilities and to develop a bond between individual and community, family, friends and significant other in their lives. More importantly, to identify those assets that most likely lead to social inclusions for a specific individual whose gifts and interests have been identified.
 
Qualifications:
Education: High School Diploma or GED required.
Experience: Experience in the human service or related field with persons experiencing disabilities desirable.
Reliable transportation, a valid driver's license, and proof of automobile insurance.
 
Compensation and Benefits:
There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Residential Direct Support Professional

Job Description
The Member Support Professional role is crucial in developing independence, building confidence, and supporting our Members towards living a full and meaningful life. The position will support adults 18+ with a variety of disabilities. 
Positions are Monday-Friday 8:30am-4:30pm and based at House of Possibilities, 350 Washington Street, Easton, MA.
In this role, you will:
● Provide the support needed by guests/members to successfully participate in community and in-house activities according to their personal needs and goals
● Assist members as needed with ADL’s and ILS
● Assist members physically as needed.
● Review assigned members and review ongoing as needed
● Dispense medications to guest per MAP regulations
● Attend weekly staff meeting
● Identify and provide the support needed by members to successfully become a part of the community and to participate in in-house activities according to each person’s
needs, abilities and wishes.
● Implement weekly schedules, menus, and activities.
● Basic Household Tasks and cleanliness of program
● If not MAP certified, must obtain certification within 3 months.
● Follow all MAP rules and regulations.
 
Other Duties
● Report any unusual incidents or behaviors to appropriate supervisory personnel and complete incident reports as necessary.
● Review and document shift notes daily for program.
 
Minimum Required Technical Skills and Qualifications
● Demonstrated background and knowledge of individuals with disabilities.
● Valid MA Driver’s license and clean driving record.
 
Education/Certifications/Licensure
● CPR/First Aid Certification
● MAP Certification. If not MAP certified on hire, then must be certified within the first 3 months of employment.
High School diploma or GED
 
Language Skills
● English

● Must be able to see and read fine print; will have close visual contact with a computer monitor constantly.
● Must be able to communicate utilizing a telephone.
● Must have the mobility to allow access to all parts of the Easton office, as well as meet with members, staff and community partners out in the community.
● Must be willing and able to learn all necessary computer functions.
● While performing the duties of this job, the employee is continuously required to sit and talk or hear, occasionally required to lift boxes weighing at least 20 lbs., and may use repetitive wrist and hand motions involved in sorting and handling documents.
● Physically assist members as necessary, including but not limited to using the restroom, vehicle transfers and wheelchair transfers.
