Provide expert nursing care in a specialized hospice environment focusing on end-of-life and complex symptom management for adult patients, primarily in home health and inpatient settings. This role requires strong assessment skills, critical thinking, and compassionate communication to support patients and their families in a meaningful way.

Key Responsibilities:

  • Deliver skilled nursing care to hospice patients in home, skilled nursing facilities, assisted living settings, and inpatient respite care.
  • Manage pain and symptom control using advanced techniques including continuous pain infusions, palliative sedation, and complex wound and ostomy care.
  • Administer medications via various routes including IV, subcutaneous, oral, sublingual, and rectal.
  • Conduct comprehensive assessments to guide individualized end-of-life care plans.
  • Facilitate clear, compassionate communication with patients, families, and interdisciplinary team members regarding goals of care.
  • Utilize clinical equipment such as CADD pumps, feeding pumps, BiPAP, CPAP, and nebulizers.
  • Participate in orientation and ongoing education including online onboarding and on-site training.
  • Rotate between home hospice visits and inpatient respite care shifts, providing consistent and reliable patient coverage.

Qualifications and Desired Experience:

  • Active RN license with 3-5 years of clinical nursing experience, including hospice or palliative care preferred.
  • BLS certification required; additional certifications encouraged.
  • Experience working in home health or hospice settings with complex symptom management strongly preferred.
  • Ability to manage a caseload of patients (typically 12-14 in home settings; smaller ratios in inpatient care).
  • Proficiency with clinical documentation systems and medication administration protocols.
  • Excellent communication skills and comfort with end-of-life discussions.

Schedule and Contract Details:

  • Contract position with 12-hour shifts at inpatient respite care and home hospice care Monday through Friday.
  • Weekend shifts required on a rotating basis.
  • Orientation lasts 2-3 days with online and in-person components.
  • Rare call requirements with potential coverage opportunities.
