This travel RN (W2) opportunity in Lawrence, Massachusetts offers a primarily weekday schedule (weekend rotation may apply by location), a supportive clinical team, and a dynamic caseload focused on start-of-care (SOC), wound care, and follow-up visits. You’ll serve patients throughout the greater Lawrence, MA area while completing high-quality OASIS documentation and coordinating care across the interdisciplinary team.\n

- Schedule: Day shift, 5x8s (08:00–17:00)\n
- Shift Duration: 8-hour days\n
- Hours per Week: 40 (productivity-based; no guaranteed hours)\n
- Caseload: Approximately 5–8 patients/day\n
- Productivity Expectation: 30 points/week (visit types and activities carry different point values)\n
- Territory: Must be willing to travel up to 75 miles from the home clinic\n
- Travel: Home visits with periodic office/clinic meetings; reliable transportation required\n
\n\nQualifications & Requirements (Registered Nurse – Home Health)\n

- Experience: Minimum 1 year of recent adult home health experience or 1 year of hospice nursing experience (as applicable to the role)\n
- License: Active Massachusetts RN license (must be in hand)\n
- Certifications: BLS/CPR\n
- Clinical Documentation: Proficiency with HomeCare HomeBase (HCHB) used within the last 6 months (or complete retraining during compliance)\n
- OASIS: RN must be proficient in OASIS documentation and used within the last 6 months (or complete retraining during compliance)\n
- Transportation: Valid driver’s license and reliable vehicle (required for home visits)\n
- Flexibility: Comfortable with variable territory routing and accepting scheduled patients as assigned\n
\n\nKey Responsibilities (Homecare / Visiting Nurse)\n
- \n
- Provide and coordinate home nursing care in accordance with physician orders and established policies/procedures\n
- Perform start-of-care assessments, wound care, follow-up visits, and ongoing patient evaluation\n
- Apply the nursing process to develop, implement, and update individualized plans of care\n
- Supervise and evaluate Home Health Aides and LPN/LVNs as assigned\n
- Complete accurate, timely documentation in HCHB; finalize documentation within 24 hours of each visit\n
- Collaborate with the clinical manager and interdisciplinary team to support quality outcomes and continuity of care\n
- Participate in periodic office/clinic meetings and support admissions across the territory as needed\n
- Meet productivity expectations and obtain pre-approval for overtime\n
