Licensed Practical Nurse (LPN) Home Health
Lexington, MA
Salary: $35$45 per hour

Responsibilities:
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor and document patient vital signs and overall health status.
- Assist with daily living activities such as bathing, grooming, and meal prep.
- Educate patients and their families on managing health conditions and following care plans.
- Coordinate with RNs, therapists, physicians, and other healthcare professionals.
- Maintain clear, accurate, and timely documentation using EHR systems.
- Provide emotional support and companionship.
- Follow all safety and health regulations in home care settings.
Requirements:
- Active and valid LPN license in the state of Massachusetts.
- Minimum 1 year of experience in home health or related healthcare setting.
- Current BLS certification.
- Reliable transportation and willingness to travel to patient homes.
- Proficient in electronic health records (EHR).
- Excellent communication and interpersonal skills.
- Strong sense of compassion and dedication to quality patient care.
