Job Details
• Helps people of all ages to participate in the activities and occupations that are important to them.
• Routinely works with individuals who have physical, cognitive, developmental, or emotional challenges that affect their ability to perform everyday tasks.
• Conduct assessments of patients' physical, mental, and developmental abilities.
• Develop individualized treatment plans based on assessments.
• Implement and monitor treatment plans, adjusting as needed.
• Provide education to patients and their families about therapeutic activities and exercises they can do at home.
• Collaborate with other healthcare professionals, such as physicians, nurses, and physical therapists, to provide comprehensive care.
• Maintain accurate and up-to-date patient records and progress notes.
• Attend staff meetings, continuing education programs, and professional development activities to stay current with new techniques and best practices.

Job Requirements
• Must have a professional license and/or certification as required for the position (MA Conditional Licenses not accepted).
• MGB Nurse Aide Registry check within 30 days of start.
• MGB Spaulding Orientation Summary required.
• Fit Test (N95) within 365 days to start date and updated annually.
• Documentation of MMR vaccination or proof of immunity.
• Influenza vaccine required during flu season (Sept 15 - April 30).
• TB Test documentation within 3 months of start date and updated if >6 month break.
• Physician Health Statement within 365 days of start date and updated if >6 month break.
• MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break.
• MGB Massachusetts CORI check within 30 days prior to start date and updated if >6 month break.
• MGB Resume including past 7 years of work experience.
• MGB Skills Checklist required for submission.
• MGB References: 2 recent references, minimum 1 Supervisor or Above, in Skillset.
• MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
• MGB Education Verification through background check company.
• MGB SSN Trace with addresses/aliases for past 7 years.
• MGB National Sex Offender Check within 30 days of start and updated annually.
• MGB Exclusion Checks within 30 days of start and annually.
