Agawam Rehab & Nursing Campus is a skilled nursing facility seeking an experienced Admissions Coordinator to fill a full-time position. As the Admissions Coordinator, you will act as a liaison to the nursing unit to help ensure a smooth transition with the admissions process for all residents.

- The Admissions Coordinator will assist with the admissions process efficiently to ensure a positive experience for residents, families, and supporting staff.
- Respond to all inquiries regarding potential residents of the facility in a sensitive, courteous, and competent manner.
- Obtain residents' insurance information, when possible, at the time a referral is called in.
- At the time of admission, meet with the resident and their families to assure all necessary forms are completed.
- Maintain admission guidelines by writing, updating, and recommending changes to admission criteria, policies, and procedures.
- Assist with patient queries and admissions procedures.
- Organize and input information to databases.
- Handling phone calls and emails from prospective patients.
Don't forget...
- Clinical experience in a skilled nursing facility setting preferred.
- Familiarity with post-acute care, rehabilitation hospitals, sub-acute rehabilitation facilities preferred.
