  1. The TCMer will be able to accurately describe targeted case management services to patients, families, and community partners.
  2. The TCMer will accurately assess the case management needs of the patient and family while completing the TCM assessment to provide the most effective services to meet the patient's and family’s needs. These needs are often diverse and will vary from one family to the next.
  3. The TCMer will provide direct and/or indirect case management services to the patient and family in the home or community settings.
  4. The TCMer will develop patient and family-driven case management goals and objectives that are consistent with the targeted case management assessment. The TCM will complete a care plan and monitor the patient's and family’s progress.
  5. The TCMer will develop and maintain a tracking system to accurately identify the case management needs of the children and families being served.
  6. The TCMer will maintain an organized system of resource materials to provide information on resources, including but not limited to: child advocacy groups, disability rights organizations, camps/recreational programs, financial assistance agencies, rent/fuel assistance programs, homeless shelters, food banks/pantries, soup kitchens, legal assistance, transportation, health/dental centers, parenting support programs/groups and other agencies as needed.
  7. The TCMer is a part of the patient's treatment team and is responsible for relaying information through the appropriate channels. This will include attendance at team meetings and consultations with other treatment providers.
  8. The TCMer is required to attend supervision, team meetings, and in-services on a regular basis.
  9. The TCMer must be aware of and adhere to all SMHC Policies and Procedures.
  10. The TCMer will be aware of and adhere to state and federal mandates regarding service provisions.
  11. The TCMer is required to maintain timely documentation in the clinical record.
  12. The TCMer will meet the clinical performance standards as developed by the agency.
  13. The TCMer will need to work a flexible, nontraditional schedule to ensure the timely provision of care.

A minimum of a bachelor's degree in human services, social work, counseling, or related field required. Experience working with children and families in the home, community, and school preferred. Experience working with systems, an understanding of family dynamics, knowledge of community and state resources, and an ability to work well within a treatment team. Knowledge of behavior management strategies is a benefit but not required. Organizational, collaborative, and advocacy skills are required. Prior experience in a mental health, human service, or school setting a plus. The Targeted Case Manager must hold a valid driver’s license and be able to prove a clean driving and criminal record. CPR and First Aid training is recommended.
