Provide expert nursing care within a specialized hospice setting focused on compassionate end-of-life support. This contract opportunity offers a diverse clinical environment balancing home hospice visits and inpatient care at a dedicated respite house. Utilize your skills in symptom management, patient education, and interdisciplinary collaboration to improve quality of life for terminally ill adults and select pediatric patients.
Key Responsibilities:
- Manage caseloads of 12-14 patients in home/SNF/ALF settings and 5-7 patients in inpatient respite care
- Perform complex symptom assessments and implement pain and medication management, including IV and subcutaneous routes
- Initiate and contribute to goals of care discussions with patients, families, and care teams
- Maintain comprehensive documentation through electronic health systems and adhere to established protocols
- Collaborate with advanced practice providers and ancillary staff to support patient needs
- Handle specialized equipment such as CADD pumps, feeding pumps, and respiratory aids
Qualifications:
- Registered Nurse with 3-5 years of clinical nursing experience, including hospice or end-of-life care
- Current BLS certification required
- Strong critical thinking and communication skills
- Ability to work independently and handle complex clinical situations
- Comfortable with rotating between different care settings and schedules
Work Schedule & Contract Details:
- 12-hour shifts from 7:00 AM to 7:30 PM inpatient; home hospice shifts Monday through Friday, 8:00 AM to 4:30 PM
- Weekend inpatient rotation required (one weekend per pay period)
- Orientation period of 2-3 days with online onboarding prior to start
