Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.
Providing compassionate, patient-centered hospice care is at the heart of this rewarding nursing role. This full-time contract opportunity involves delivering skilled end-of-life care to patients in their homes, assisted living, or nursing facilities within the East Falmouth, MA area. The position focuses on enhancing comfort, dignity, and quality of life for patients facing terminal illness, requiring both clinical expertise and deep empathy.
Key Responsibilities:
- Perform thorough nursing assessments and develop personalized hospice care plans.
- Administer medications and treatments aligned with physician orders and hospice protocols.
- Manage symptom control and pain relief to ensure patient comfort.
- Monitor patient condition changes and communicate effectively with the hospice medical director and care team.
- Educate patients, families, and caregivers on disease progression and supportive care techniques.
- Collaborate with interdisciplinary team members including social workers, chaplains, and aides.
- Document all patient care accurately using the Home Care Home Base (HCHB) electronic system.
- Participate in team meetings and quality improvement activities.
- Adhere to hospice policies and regulatory guidelines.
Qualifications:
- Active RN license in Massachusetts, in good standing.
- Minimum of one year recent hospice nursing experience.
- Proficiency with Home Care Home Base (HCHB).
- Strong clinical assessment and communication skills.
- Reliable transportation with valid drivers license and auto insurance.
- Ability to manage a field-based nursing schedule independently.
- Compassionate demeanor with emotional resilience.Preferred:

- Experience in palliative or end-of-life care.
- Knowledge of Medicare hospice regulations and documentation standards.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM, with occasional weekend or on-call rotations.
- Approximately 45 patient visits per day with daily travel within assigned service area; mileage reimbursement provided.
