Our ambulatory registered nurses (RN’s) provide patient-centered care in an adult outpatient primary care setting, supporting patients and families ensuring high-quality, coordinated care focusing on prevention, chronic disease management and health promotion.
Responsibilities
- Systematically collect and assess patient health data (via phone, patient portal or in person) to develop, implement and evaluate individualized care plans.
- Coordinates care among various healthcare providers ensuring patients receive appropriate services and follow up, and facilitating smooth transitions between care settings.
- Provide education for patients and families about health conditions, treatment plans, disease prevention, and self-care strategies to empower them in managing their health
- Advocate for patient needs, provide emotional support, and ensure equitable access to care and resources.
- Delegate tasks appropriately to licensed and unlicensed personnel.
- Collaborate closely with other care team members (physicians, residents, advance nurse providers) to deliver care that addresses all aspect of the patient’s health.

Essential Functions
- Maintain accurate, detailed reports, and records.
- Administer medications to patients and monitor patients for reactions or side effects.
- Record patients' medical information and vital signs.
- Monitor, record, and report symptoms or changes in patients' conditions.
- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
- Modify patient treatment plans as indicated by patients' responses and conditions.

Education
Associate's Degree Nursing Required Or Bachelor's Degree Nursing Preferred

Licenses and Credentials
Registered Nurse [RN - State License] - Generic - HR Only preferred
Experience
Clinical nursing experience 0-1 year required
Knowledge, Skills And Abilities
- Familiarity with the principles and skills needed for practical nursing to provide patient care and treatment.
- Knowledgeable of the care required by respective age groups for which care is being provided.
- Ability to maintain confidentiality and secure sensitive information.
- Knowledge of medical terminology.
- Excellent verbal and communication skills.
- Ability to accurately screen and triage acute patients.
