We are currently hiring a Care Manager.

As a Care Manager, you will assess and monitor the needs of the consumer and implement a service plan which is following the Executive Office of Elder Affairs and Elder Services of Cape Cod and the Islands Home Care

Program regulations and contract obligations. This is a full-time position, Monday- Friday, 9-5, 34 hours/ week plus unpaid one hour lunch each day. The pay rate for this position is $28.43 per hour.

What do you need to be considered for this position?

  • Ability to collaborate, take initiative, provide/ receive feedback.
  • Ability to communicate and work with the elderly and community resources serving the elderly population.
  • Excellent computer skills required. Ability to work independently and professionally.
  • Valid MA driver’s license and car required.
  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds.
  • Travel Required daily throughout the service area. Travel to training and meetings as needed.
