This nursing opportunity offers specialized experience in hospice and end-of-life care within a supportive home health and inpatient setting. Serving a diverse adult patient population facing terminal illnesses, candidates will apply advanced nursing skills to manage complex symptoms and provide compassionate care. The role is contract-based and requires flexibility to rotate between home hospice visits and inpatient care at a dedicated respite house.
Responsibilities:
- Manage a caseload of approximately 12-14 patients in home settings or 5-7 patients in an inpatient respite house during day shifts; overnight shifts may involve up to 11 patients
- Perform comprehensive patient assessments and develop care plans focused on comfort and symptom management
- Administer medications via multiple routes, including IV, subcutaneous, oral, and others, adhering to strict protocols
- Initiate and participate in goals of care discussions with patients, families, and interdisciplinary teams
- Utilize clinical judgment to apply pain management techniques, wound care, central line maintenance, and palliative sedation
- Document patient care using a specialized electronic health record system
- Collaborate with CNAs, PCTs, and nurse practitioners supporting clinical activities
- Occasionally cover weekend rotations, traveling across a multi-county service area
Qualifications:
- Active RN license with 3-5 years of clinical nursing experience; hospice/end-of-life specialty experience preferred
- Valid Basic Life Support (BLS) certification
- Strong critical thinking and communication skills
- Ability to work independently in both home and inpatient environments
- Comfortable with technology including portable medical devices and electronic documentation systems
