JOB DESCRIPTION

POSITION TITLE:

Nurse Wellness Educator

POSITION SUMMARY:

Under the supervision of the Chief Medical Officer, plans, develops, and implements diabetes education programs for both individual patients and groups. Develops and implements protocols for administering Medicare Initial Preventive Physical Exams (IPPEs) and Annual Wellness Visits (AWVs). Collaborates with Patient-Centered Medical Home (PCMH) committee to prepare annual submission to National Committee for Quality Assurance (NCQA) for re-certification.

PRIMARY RESPONSIBILITIES:

  1. Provides education and nutritional assessments for patients with diabetes.
  2. Collaborates with patients to develop individualized diabetes care plans.
  3. Conducts group visits for diabetes education, nutrition education, or other educational topics related to patient health.
  4. Develops and implements protocols for administering Medicare IPPEs and AWVs.
  5. Trains providers to administer Medicare IPPEs.
  6. Administers AWVs in coordination with primary care teams.
  7. Collaborates with work group tasked with maintaining adherence to PCMH policies and procedures.
  8. Assists with preparation and submission of documents for annual NCQA re-certification.
  9. Attends health center staff clinical team meetings and other staff meetings as required. Participates in development of group practice standards, quality assurance objectives and activities, and presents didactic teaching material to other health center clinical team members, as requested by CMO.
  10. Develops culturally appropriate educational materials on diabetes and nutritional issues for use with individual patients and community health education programs. Evaluates and recommends diabetes and nutrition education used by other health center staff.
  11. Documents all patient care, including face-to-face and telephone encounters and consults, appropriately and legibly in the medical record per health center protocols.
  12. Seeks appropriate consultation and clinical supervision as needed from colleagues, including transfer of patients exceeding professional skills, abilities, or comfort.
  13. Provides complete, accurate billing data for all encounters in a timely manner per health center policy.
  14. Maintains all health center required credentials at all times and provides health center with updated copies. Maintains and expands current level of professional knowledge of diabetes care and nutrition theory and practice through continuing education.
  15. Other duties as required and assigned.

Location: Attendance is required during regular business hours at assigned CHCFC site(s). Remote work may be arranged by approval of CMO, when appropriate.

Other: Bilingual/bicultural strongly preferred; commitment to CHCFC’s mission and ability to work with diverse medically underserved population required.

REQUIRED SKILLS, EDUCATION & EXPERIENCE:

Education: Certified Diabetes Educator/CDE and Registered Nurse (RN) required. Bachelor’s degree preferred.

Experience: Two or more years in public health or office nutritional counseling strongly preferred.

Skills: Strong knowledge of dietary counseling regarding chronic disease factors (cardiovascular, hypertension, diabetes) and obesity required. Strong teamwork and community activism skills.

The work requires some travel to and from evening clinics and training sessions as well as the ability to adjust work schedule to accommodate periods of extreme activity.

WORKING CONDITIONS:

While performing the required duties of this job, the employee is exposed to weather conditions prevalent when traveling between Health Center locations where medical care is provided. This position performs tasks that involve exposure to blood, body fluids or tissues.
