Patient Care Manager / Registered Nurse, Home Health
Location: Norwood MA
Shift: Day shift, M-F
Position Summary:
We are seeking a highly skilled and compassionate Patient Care Manager / Registered Nurse (RN) to join our Home Health team. The ideal candidate will be a dynamic leader with a strong clinical background and a passion for providing exceptional patient care in the home setting. This role is responsible for the overall management and coordination of patient care services, ensuring the delivery of high-quality, safe, and effective care that meets regulatory standards and patient needs.
Key Responsibilities:
- Clinical Oversight & Management:
- Supervise and mentor a team of home health nurses, certified nursing assistants (CNAs), and other caregivers.
- Review and approve patient care plans, ensuring they are comprehensive, accurate, and aligned with physician orders.
- Conduct regular chart audits and quality assurance checks to ensure compliance with company policies, state and federal regulations (e.g., Medicare, Medicaid), and professional standards.
- Serve as a clinical resource for staff, providing guidance and support on complex patient cases, wound care, and disease management.
- Coordinate with physicians, therapists, and other healthcare providers to ensure a holistic approach to patient care.
- Patient & Family Communication:
- Act as the primary point of contact for patients and their families, addressing concerns, providing education, and fostering a trusting relationship.
- Conduct initial patient assessments and ongoing evaluations to determine eligibility for home health services and to monitor patient progress.
- Communicate changes in patient condition to physicians and the interdisciplinary team in a timely and effective manner.
- Administrative & Operational Duties:
- Assist with the recruitment, onboarding, and ongoing training of clinical staff.
- Participate in the development and implementation of clinical policies and procedures.
- Manage and optimize patient scheduling to ensure efficient and timely delivery of care.
- Maintain accurate and up-to-date patient records and documentation.
- Participate in on-call rotation as needed.
Qualifications:
- Bachelor of Science in Nursing (BSN) required; Master's degree in Nursing (MSN) or related field a plus.
- Current and unrestricted Registered Nurse (RN) license in [State Name].
- Minimum of [Number] years of clinical nursing experience, with at least [Number] years in a home health setting.
- Proven leadership and supervisory experience in a healthcare environment.
