Overview
We are seeking a dedicated and compassionate Nurse Practitioner to join our patient-centered, Family Practice and Lifestyle Medicine focused medical practice. The ideal candidate will provide high-quality care across all ages, with a strong emphasis on Lifestyle Medicine—including nutrition, physical activity, sleep, stress management, substance use reduction, and social connection. This role requires a deep commitment to preventive care, patient advocacy, and collaborative, team-based healthcare.
Duties
- Conduct comprehensive patient assessments and physical examinations for pediatric, adult, and geriatric patients, with a focus on root-cause evaluation and long-term wellness.
- Develop and implement individualized treatment plans that integrate evidence-based primary care and lifestyle interventions alongside traditional medical therapies.
- Administer medications and monitor patient responses as part of an integrated, family-centered approach to care.
- Provide chronic disease management and support, with an emphasis on patient education, empowerment, and prevention.
- Coordinate follow-up care and ongoing management plans that highlight patient self-management and healthy lifestyle practices.
- Address behavioral health concerns and refer to appropriate specialists, recognizing the impact of mental well-being on family and community health.
- Educate patients and families on nutrition, exercise, sleep, stress reduction, and other lifestyle factors as part of routine preventive and acute care.
- Maintain compliance with HIPAA regulations and uphold standards of ethical and evidence-based practice.
- Utilize eClinicalWorks for efficient, accurate documentation and to track patient progress toward health and lifestyle goals.
- Provide family medicine–based preventive screenings, immunizations, and wellness counseling to patients of all ages.
Skills
- Strong foundation in Primary Care with interest in Lifestyle Medicine principles and ability to apply them across a wide range of family medicine scenarios.
- Proficiency in medication administration and related safety standards.
- Experience managing chronic conditions (e.g., diabetes, hypertension, obesity) with an emphasis on lifestyle-driven care.
- Knowledge of preventive care, pediatric and adult wellness visits, and chronic disease support with a lifestyle-centered perspective.
- Understanding of behavioral health and its interconnection with physical and social well-being.
- Excellent communication skills to engage, motivate, and educate patients and families.
- Ability to perform routine family medicine procedures and supportive care techniques.
- Commitment to patient confidentiality and adherence to HIPAA and medical ethics.
