Pay: $20.00-$20.50 per hour based on certifications

SUMMARY OF POSITION:
To provide one on one direct support care to individuals who live independently and reside in community-based settings. To ensure the health and safety of all individuals and promote independence in all areas of daily living. To include but not limited to a focus on community integration, health and wellness, and skill development.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provides supervision, assistance, and support to individuals receiving services from Almadan, Inc. in accordance with the individuals support plan
Ensure the health and safety of all individuals receiving services
Ability to assist individuals as needed to promote independence in activities of daily living skills, to include but not limited to: Community integration, meal preparation, funds management, and social skills, Personal care, Maintaining a clean and safe home environment
Review and complete all required daily documentation
Assist Individuals to navigate services, including housing, healthcare, employment and financial resources.
Provides transportation to individuals in accordance with agency policy
Maintain current certification and training in CPR, First Aid, Nurses Training as well as any required site-specific trainings
Perform job responsibilities in accordance with agency policy and procedure manual in addition to state regulations
Attend and participate in quarterly staff meetings in accordance with agency policy
Attend and participate in mandatory meetings identified by the direct supervisor and in accordance to agency policy
Participate in the development of individuals ISP under the direction of the Program Director/Manager;
Ensure implementation of provider support strategies outlined in individuals ISPs
Maintain a flexible schedule as schedules may be altered based on the needs identified by individuals receiving supports
Perform other duties as assigned.
MINIMUN ACCEPTABLE QUALIFICATIONS:
High School Diploma
One year of experience working with the DDS/DMH population preferred
Must be 18+ years of age
Must have a valid drivers license and reliable vehicle to transport individuals
MARGINAL DUTIES AND RESPONSIBILITIES:
Ensure duties and responsibilities are completed in accordance with agency policy and procedure
Attends appropriate meetings as required
Other job-related duties, as requested
PHYSICAL AND MENTAL REQUIREMENTS:
Effective written and verbal communication skills.
Must be able to bend, lift, stoop, and reach
Must be able to switch from tasks to task (flexible) during the work day and work cooperatively with Agency staff.
