Job DescriptionRegistered Nurse (RN) – Home Health (Travel Assignment)

Location: South Dennis, MA 02660
Assignment Length: 13 Weeks
Pay Rate (Travel): $70.00 – $74.00 per hour
Employment Type: Travel Contract (Travelers Only)

Schedule

  • Primary schedule: Monday – Thursday

  • May extend to Monday – Friday (40 hours/week) as needed

  • Weekend rotation approximately every 4 weeks

  • Occasional holidays required based on patient care needs

Position Summary

"Navitas Healthcare, LLC" is seeking Registered Nurse (RN) – Home Health for an exciting job in South Dennis, MA.

Key ResponsibilitiesPatient Assessment & Care Planning

  • Conduct comprehensive admission assessments, including physiological, psychosocial, environmental, and health history evaluations.

  • Identify patient problems and develop individualized plans of care.

  • Initiate, implement, revise, and document care plans based on ongoing patient needs.

Clinical Outcomes & Case Management

  • Maintain Home Health Resource Group (HHRG) values at national standards.

  • Utilize HHRG data to determine visit frequency and interdisciplinary resource utilization.

  • Ensure patient outcomes consistently meet or exceed national benchmarks.

Coordination & Collaboration

  • Participate in interdisciplinary case conferences to evaluate and adjust plans of care.

  • Communicate significant changes in patient condition to physicians and care team members.

  • Refer patients to appropriate community resources when indicated.

Supervision & Education

  • Provide instruction and documented supervision of Home Health Aide care plans every 14 days.

  • Deliver patient and caregiver education and document understanding and response.

Documentation & Compliance

  • Complete all admissions, visit notes, and required documentation accurately and on time.

  • Maintain detailed narrative documentation reflecting skilled nursing care.

  • Obtain, verify, and clarify valid physician orders for all services rendered.

Discharge Planning

  • Coordinate and plan appropriate patient discharges.

  • Communicate discharge plans with patients and physicians.

  • Complete all required discharge documentation and summaries.

  • Perform additional duties as assigned.

Qualifications & Requirements

  • Active Massachusetts RN license

  • Successful completion of required pre-hire medication assessment

  • Minimum 1 year of nursing experience, or 6 months with a BSN

  • CPR certification required

  • Home Health and IV experience preferred

  • Strong computer and documentation skills

  • BSN preferred

  • Travelers only
