We are currently hiring a part time BCBS Transition Coach. The BCBS Transition Coach will conduct visits, complete assessments and provide support to BCBS patients transitioning from an inpatient setting to another setting.
Monday – Friday, 20.00 hours per week at 30.42 per hour.
What do you need to be considered for this position?
- Ability to collaborate, take initiative, provide/ receive feedback.
- Excellent computer skills required. Ability to work independently and professionally.
- Must possess knowledge of long-term care, case management, discharge planning, community resources, programs, and benefits to help support an individual’s transition from an institutional to a community setting.
- Valid MA driver’s license and car required
- Associates or Bachelors Degree in related field preferred
- Prolonged periods sitting at a desk and working on a computer. Must be able to lift 15 pounds.
