Alfa Care LLC is a growing home health agency serving individuals and families across Hillsborough County, New Hampshire. We are urgently hiring compassionate and dependable Licensed Nursing Assistants (LNAs) to provide quality personal care and support services to clients in their homes.
This role is ideal for LNAs who enjoy one-on-one patient care, consistent assignments, and working with a supportive clinical team.
What You’ll Do
Under the supervision of a Registered Nurse, you will:
- Assist clients with ADLs and iADLs according to the Plan of Care (POC)
- Support personal hygiene, mobility, nutrition, and safety needs
- Take and document vital signs as directed
- Assist with ambulation and transfers per care plan
- Observe and promptly report changes in patient condition (including skin changes)
- Encourage independence, engagement, and emotional well-being
- Maintain a safe, clean, and comfortable home environment
- Document visits accurately and on time
- Work collaboratively with nurses and the interdisciplinary care team
Qualifications
- Active NH LNA license or completion of a state-approved nurse aide or home health aide program
- Ability to read, write, and communicate effectively in English
- CPR certification
- Ability to pass a criminal background check
- Reliable transportation for local home visits
- Compassionate, dependable, and professional demeanor
Work Environment & Physical Requirements
- Care provided in client homes
- Local travel required
- Ability to lift, transfer, and assist patients as needed
- Ability to access homes that may not be handicap accessible
- Possible exposure to communicable diseases
