Job Summary
We are seeking a Care Coordinator to join our Greater Horizon Healthcare team. In this role, you will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices.

ESSENTIAL DUTIES AND RESPONSIBILITIES


  • Promotes optimal health for individuals though routine, preventive, and emergency health care. Ensures that work environments and Life Enhancement activities are safe, secure and where individuals are free from harm.
  • Develop, write and present annual Individual Support Plans (ISPs) and the subsequent support paperwork in consultation with their supervisor.
  • Performs case management duties including documentation of training and activities, maintenance of individual confidential files on assigned caseload, review and submit all required paperwork to supervisor according to agency procedures. Attends regular meetings as necessary.
  • Ability to be flexible regarding necessary changes in work schedule and tasks as assigned by supervisor to cover the needs of the individual supported.
  • Ability to demonstrate good computer skills as well ability to learn to utilize the agencys computer programs for documentation of necessary data.
  • Other duties and/or assignments to work/program locations based on the needs of the company.

QUALIFICATIONS & REQUIREMENTS

  • Previous experience as a Care Coordinator or in a similar position is preferred
  • Certification as a medical assistant is preferred
  • Strong problem-solving and organizational skills
  • Ability to manage multiple projects or tasks and prioritize appropriately
  • Ability to work in fast-paced situations and make sound decisions quickly
  • Excellent interpersonal skills and high level of compassion
  • Strong verbal and written communication skills
  • Valid driving license
  • Must be a US Citizen or a Green Card Holder
  • Valid CPR, First Aid and other relevant certifications

EDUCATION and/or EXPERIENCE

  • BA/BS in Human Services /related field or associate degree or HS Diploma/GED plus 4-5 years of related experience
  • 2-3 years related experience with persons with ID/DD or Human Services

LANGUAGE SKILLS

  • Able to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
  • Able to write and proofread routine reports
  • Ability to speak English (Multi-lingual will be preferred)

VEHICLE/TRAVEL

  • Requires a legally insured and inspected automobile for use to be presented at work
