Sunbelt Staffing is currently hiring a compassionate and experienced Registered Nurse (RN) for a 13-week home health contract assignment in Bedford, NH. In partnership with a respected home health agency, this position offers the opportunity to deliver personalized, high-quality care directly in patients homes. As a vital member of the care team, youll help support patient independence, improve outcomes, and make a meaningful difference in their daily lives.

Contract Details:

  • Location: Bedford, NH,and surrounding areas
  • Company: Sunbelt Staffing in partnership with a nationwide home health/hospice agency
  • Contract Length: 13 Weeks (with possible extension upon successful start and completion)
  • Hours: Full-time, 40 hours per week, Monday-Friday, 8am-5pm, with occasional weekends and on-call duty (typically one weekend per month on-call)
  • Productivity: 25-32 per week, up to 35 patients per week
  • Compensation: Negotiable, based on experience and territory budget
  • Mileage Reimbursement: .48c from patient home to home, 150-250 miles per week expected
  • Blended Pay Packages: Available for travelers
  • Benefits: Medical, dental, vision, 401k match after 90 days

Job Responsibilities

  • Perform comprehensive nursing assessments for hospice patients in their homes, documenting all findings accurately in Homecare Homebase (HCHB).

  • Develop and implement individualized, compassionate end-of-life care plans in collaboration with physicians, social workers, chaplains, and other members of the hospice team.

  • Provide skilled nursing care including pain and symptom management, medication administration, and emotional support tailored to patients terminal diagnoses.

  • Offer guidance and education to patients and their families on the disease process, comfort measures, and caregiving techniques.

  • Maintain timely and thorough documentation to ensure compliance with agency protocols, Medicare regulations, and Joint Commission standards.

  • Travel to patient residences within the designated territory, using reliable transportation.

  • Participate in regular interdisciplinary team meetings and case conferences to coordinate holistic, patient-centered care.

  • Ensure all care delivered aligns with hospice philosophy, prioritizing dignity, comfort, and quality of life.

Requirements

  • Active Registered Nurse (RN) license in the state of New Hampshire.
  • Minimum of 1 year of hospice experience in the home setting.
  • Proficiency in Homecare Homebase (HCHB).
  • Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.
  • Reliable transportation with valid driver's license.
  • Ability to meet Joint Commission onboarding standards upon offer.
