Job Details
• Seeking a Manager of Case Management position with leadership experience.
• Urgent need for a manager with 2 years relevant management/supervision experience along with acute hospital experience.

Job Requirements
• Must have 2 years relevant management/supervision experience.
• Acute hospital experience required.
• Conditional license accepted if permanent licensure process has begun.

Schedule Information
• Contract duration options: 13 or 20 weeks.
• Work schedule: 8am-4:30pm, 5 days a week (5x8).

Additional Details
• Submit a recent color photo (no selfies, no filters, plain background, no eyeglasses).
• Primary source verification of professional license/certification required within 30 days of start date.
• N95 fit test required within 365 days of start date, updated annually.
• Documentation of MMR vaccination or immunity required.
• Hepatitis B vaccination or immunity preferred.
• Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions.
• TB skin test or equivalent required within 3 months of start date, updated if break >6 months.
• Physician statement required within 365 days of start date, updated if >6 month break.
• MGB criminal background check and Massachusetts CORI required within 45 days prior to start date.
• Resume including past 7 years of work experience required for submission.
• References required (minimum 2, at least 1 supervisor or above).
• Skills checklist required for submission.
• Confirmation of assignment/POI form required for all initial assignments and extensions.
• Education verification required.
• Social Security Number trace required.
• National sex offender check required.
• Exclusion checks required (OIG, GSA, MassHealth, NH Medicaid).
• TB symptom survey required if positive PPD.
• Candidate submission coversheet required for submission.
