ABOUT THE ROLE:

The Director of Population Health & Quality will be responsible for the development, coordination, communication, and implementation of quality and population health improvement programs and initiatives. The position will develop and manage Community Health’s improvement projects, including ensuring impact at a population level and identifying and prioritizing initiatives to align with the needs of the community. The position will oversee the development and implementation of quality & population health analytics. The position will work with cross-functional teams to create streamlined information flows and closed-loop processes to identify potential patient needs and risk factors to deliver needed care and close gaps to achieve critical population health outcomes and targets. The position will oversee prevention and wellness programming.

FUNCTIONS OF THE POSITION:

  • Directs and providers leadership for implementing, monitoring, and evaluating quality management and population health programming.
  • Provides leadership for the interpretation of results and development of improvement action plans arising from provider, staff, and patient satisfaction surveys.
  • Prepare reports and analysis on population health and quality metrics that inform the organization’s care, utilization, and patient outcomes.
  • Articulate strategy and clarify path to achieving quality targets by contract by ensuring accurate documentation of patient needs and risk factors, accounting for key drivers of medical expense, and reflecting these needs in a coordinated approach to gap closure
  • Work with Clinical lead to create clinician education materials and coach clinicians on gap closure and documentation, including joining routine quality reviews as needed and identifying issues in gap closure performance.
  • Works in collaboration with the Director of Care Management in the implementation of and sustainment of healthcare initiatives and incentive programs (PCMH, Chronic Care Coordination, Quality Initiatives, value based & collaborative care models, and) that promote population health management, patient satisfaction and revenue optimization associated with improving clinical outcomes of the patient population we serve.
  • Coordinates the process of monitoring, measuring, and assessment of patient care and support of systems to achieve high quality, safe, cost-effective healthcare services.

SKILLS REQUIRED FOR SUCCESS:

  • Bachelor of Science in Nursing or Clinical Field required; Master’s in Public Health, Nursing, Healthcare Administration, or similar discipline preferred.
  • Candidate must have five to seven years’ experience in the following areas: extensive experience with healthcare data; a comprehensive knowledge of healthcare operations, project leadership in a complex healthcare work environment with ability to identify opportunities for improvement and develop solutions, experience working with Electronic Health Records, clinical databases and registries. Must be able to lead teams toward common organizational objectives to achieve strategic goals.
  • Experience and understanding of population health management, clinical quality improvement process, data analysis, outcome management and disease management.
  • Experience designing clinical dashboards, designing, and producing report templates and reports to meet clinical and regulatory requirements.
