This role involves managing personalized, outcome-focused nursing care through the nursing process. Responsibilities include providing care, treatment, and services by effectively coordinating and completing a series of processes such as assessment, care planning, delivering care, coordinating care across the continuum, and evaluating the outcomes of interventions in line with organizational, departmental, and ambulatory policies and procedures.

The position emphasizes clinically-based, risk-reducing care and coordination that promotes the delivery of cost-effective, high-quality healthcare throughout the continuum. It requires direct interaction with physicians, care managers, healthcare teams, patients, and their caregivers to achieve optimal patient outcomes. Strong communication skills, both written and verbal, a dedication to coaching patients on health-promoting behaviors, and the ability to comprehend and integrate complex clinical information are essential. The capacity to work independently as well as collaboratively in teams is crucial. Participation in quality improvement initiatives, research, and evaluation processes related to patient care management is a key aspect of this role.

ESSENTIAL FUNCTIONS

1 - Medication Administration

  • *Exhibits knowledge of and adheres to organizational policies and procedures for administering and documenting medications.
  • *Completes the medication reconciliation process in accordance with organizational policy.
  • *Demonstrates safe practices for administering medications and vaccines.

2 - Plan of Care/Documentation/Patient Family Centered Care/Patient Experience

  • Creates, discusses, and communicates a prioritized problem list and care plan for each patient.
  • Develops, assesses, and updates individualized care plans and documents outcomes.
  • Initiates patient outreach, identifies and documents needs of patients/families/significant others, and implements appropriate interventions while evaluating their effectiveness.
  • Continuously assesses and reassesses to identify care needs within established nursing practices.
  • Documents all patient care in accordance with nursing department policies, standards, disease processes (e.g., CHF, PNA vaccines), and nurse-sensitive indicators (e.g., falls, safety risks, isolation).
  • Assesses and documents patient responses to both pharmacological and non-pharmacological interventions per organizational policies.
  • Advocates for patients/families, seeking assistance to represent their needs when necessary.
  • Raises ethical concerns with the clinical team and seeks resources to navigate ethical decisions.
  • Actively communicates and collaborates with health team members, including primary care, specialty care, and ancillary providers.
  • Facilitates care across the continuum to ensure patient needs are met in the appropriate setting and timely manner.

3 - Safety/Quality

  • Promotes a "Culture of Safety" through personal accountability and commitment to a secure environment.
  • Confirms patient identification using two identifiers before initiating any procedure, including "time out," administration of care, medications, labeled specimens, and documentation to ensure accuracy.
  • *Adheres to current CDC hand hygiene guidelines through proper handwashing and follows universal precautions, utilizing personal protective equipment appropriately and disposing of hazardous materials correctly.
  • Ensures the environment consistently meets regulatory standards.
  • Understands and can demonstrate individual roles and responsibilities during ambulatory codes/emergency preparedness.

4 - Professional Development

  • Takes responsibility for personal professional growth by integrating evidence-based practices, research, and performance improvement initiatives into ongoing nursing practice.
  • Obtains a minimum of 5 contact hours annually in the area of practice.
  • Operates within the legal boundaries of the Nurse Practice Act and directs licensed and non-licensed personnel as assigned.
  • Shows the ability to accept and respond constructively to feedback and suggestions for improvement.
  • Fosters professional relationships with colleagues across all disciplines to create a supportive workplace.
  • Employs critical thinking to analyze problems, evaluate alternative solutions, and select appropriate actions while effectively communicating through the chain of command.
  • Stays informed about changes in policies and procedures.

5 - Technology

  • Utilizes technological solutions to enhance work processes and practices.
  • Accesses Outlook, Epic, HealthStream, and Workday for email, learning management, and other resources relevant to the RN role.
  • Utilizes software applications required by the health system, nursing, and departmental standards.
  • Understands how to operate during downtime.

6 - Compliance

  • Operates within legal, regulatory, and ethical standards pertinent to the position.
  • Adheres to applicable policies and procedures.
  • Protects the privacy and security of patient information, complying with organizational privacy and security protocols.
  • Reports potential compliance issues through the appropriate chain of command.
  • Completes mandatory education requirements related to compliance, privacy, and security programs.

JOB REQUIREMENTS

Minimum Education - Preferred

  • Graduate of an accredited School of Nursing; BSN preferred.

Minimum Work Experience

  • Previous experience as an Ambulatory Care RN is preferred.

Required Licenses / Registrations

  • RN - Registered Nurse.

Required Classes/Skills

  • BLS – Basic Life Support.

Additional Knowledge and Abilities Required

  • Proficiency in ambulatory care nursing, including relevant knowledge and skills.
  • Excellent verbal and written communication skills are essential.
  • Demonstrates adaptability to changing priorities and regulations.
  • Basic computer skills are necessary.
  • Exhibits skills in communication (both verbal and written), interdisciplinary collaboration, creative problem-solving, and critical thinking.
  • Knowledge of health system and community resources is preferred.
