As an LPN, you’ll play a vital role in supporting our patients and RN Case Managers by:

Following the individualized plan of care and physician’s orders to deliver safe, high-quality nursing care.

Teaching patients and families simple self-care techniques to support independence.

Monitoring patient progress, reporting changes in condition, and suggesting updates to care plans.

Providing positive, compassionate communication to patients, families, and caregivers.

Helping prevent unnecessary ER visits or hospitalizations by delivering proactive care at home.

Graduate of an accredited LPN/LVN program

Current State License (LPN/LVN)

Minimum 1 year of clinical experience (home health or hospice a plus)

Reliable transportation, valid driver’s license, and insurance

Ability to lift 50–100 lbs as needed
