The Young Adult Community Case Manager provides care planning, resource identification, benefit coordination, service monitoring, and community-based counseling support to assist clients with improving symptoms and behaviors that otherwise interfere with age-appropriate development, functioning and wellness goals. QUALIFICATION REQUIREMENTS

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
<ul>
<li>Develop, implement, and oversee infusion plans of treatment, evaluating patient progress and modifying care as necessary.</li>
<li>Perform comprehensive patient assessments, prescribe medications, and manage infusion therapies in alignment with state scope of practice and collaboration agreements.</li>
<li>Provide direct patient care, including physical assessments, lab test interpretation, medication prescriptions, and infusion administration.</li>
<li>Educate patients and caregivers on infusion treatments, ensuring patient safety and adherence to therapy.</li>
<li>Coordinate patient scheduling, oversee staffing, and optimize workflow for clinical operations.</li>
<li>Maintain accurate and timely clinical documentation in the electronic medical record (EMR).</li>
<li>Monitor and manage medication and supply inventory per clinical guidelines.</li>
<li>Ensure compliance with federal, state, and local regulations, including HIPAA and payer credentialing requirements.</li>
<li>Collaborate with referring providers, clinical staff, and administrative teams to optimize patient care.</li>
<li>Lead quality assurance activities, accreditation compliance, and process improvement initiatives.</li>
<li>Provide mentorship and supervision to clinical staff, including RNs, LPNs, and administrative personnel.</li>
<li>Participate in staff training, orientation, and professional development programs.</li>
<li>Respond promptly to patient concerns and medical emergencies within scope of practice.</li>
<li>Oversee multi-site clinical operations as needed and ensure adherence to best clinical practices.</li>
<li>Supervision of Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and clinical administrative staff.</li>

KNOWLEDEGE, SKILLS AND ABILITIES REQUIREMENTS
<ul>
<li>Clinical Expertise – Extensive knowledge of infusion therapy, specialty medications, and management of patients with neurological, rheumatologic, gastroenterological, and immunological disorders in an Ambulatory Infusion Suite/Clinic setting.</li>
<li>Patient-Centered Care – Ability to assess, diagnose, and manage patients while providing high-quality, evidence-based care with a focus on patient safety, education, and satisfaction.</li>
<li>Regulatory Compliance – Understanding of state and federal regulations, including HIPAA, OSHA, and infusion therapy protocols, to ensure adherence to industry and company standards.</li>
<li>Pharmacology &amp; Medication Administration – Proficiency in prescribing, preparing, and administering intravenous (IV) and subcutaneous medications, including biologics, immunotherapies, and specialty drugs.</li>
<li>Critical Thinking &amp; Decision-Making – Strong analytical and problem-solving skills to evaluate patient responses, adjust treatment plans, and manage adverse reactions effectively.</li>
<li>Leadership &amp; Team Collaboration – Ability to oversee clinical staff, provide mentorship, and work collaboratively with interdisciplinary teams to ensure optimal patient outcomes.</li>
<li>Technology &amp; EMR Proficiency – Skilled in using electronic medical records (EMR) systems for patient documentation, scheduling, and reporting, as well as other clinical software and equipment.</li>
<li>Communication &amp; Interpersonal Skills – Strong verbal and written communication skills to interact with patients, caregivers, physicians, and healthcare teams with clarity and professionalism.</li>
<li>Time Management &amp; Multitasking – Ability to prioritize and manage multiple patients, treatments, and administrative tasks efficiently in a fast-paced clinical setting.</li>

<li>Adaptability &amp; Initiative – Proactive in identifying opportunities for improvement, implementing best practices, and staying current with industry advancements in infusion therapy and ambulatory care.</li>
