The Property Manager – Residential works under the direction of the Assistant Director of Residential Services, to manage properties for young people experiencing out of home placement. This position will ensure that all property, municipal, regulatory, and contractual requirements are met. Duties will include, managing all necessary administrative tasks, manage maintenance ticket workflows through project completion, small repairs, and identifying/assessing/coordinating the need for property repairs and maintenance on an ongoing basis. This position will develop and maintain strong relationships with Program Managers, vendors &amp; contractors and LUK’s Finance department. The essential responsibilities of the position include, maintaining documentation of all communication and services provided, conducting regular property inspections and to provide support &amp; instruction to staff and clients to develop skills to maintain safe and presentable living spaces.
 
The qualified candidate will have at least 2 years of property management experience. Excellent communication, planning, organization, and computer skills required. Valid driver’s license and own transportation required.

Specific Duties and Responsibilities:
<ul>
<li>Assist individuals with planned activities and community involvement.</li>
<li>Have a strong personal boundary and not share information of family, children, etc.</li>
<li>Develop a relationship map to gather information about the individual in order to establish social inclusion opportunities.</li>
<li>Assist individuals in developing a network of people in their lives. Maximize opportunities for the individuals they support with disabilities to get to know others and maximize the opportunities for others to get to know the individual.</li>
<li>Assist the individual in obtaining a sense of self-esteem through active participation in the community. </li>
<li>Provide individualized type and level of support to include, but not be limited to, personal care and/or skill acquisition etc.</li>
<li>Assist individuals in establishing, maintaining, and presenting a positive image. This may involve such activities as assisting in the bathroom, bathing, showering, and dental hygiene, etc.</li>
<li>Maintain a calm demeanor and respond quickly and appropriately to aggressive or verbal outbursts. Scan room and individual for potential danger.</li>
<li>Assist in developing and individualizing the yearly Person Centered Planning by providing information for assessments and by developing implementation strategies for goals/objectives and methods for monitoring the delivery and quality of supports to the individual.</li>
<li>Complete documentation requirements as specified by the Individual Service Plan to include the scheduled data collection and monthly progress notes to show an increase or decrease in behaviors to continually support individuals as they grow and reach goals.</li>
<li>Assist in the orientation/training of new staff, which may include introduction to an individual's support persons and to an individual's ISP, strategies, dreams and goals, and treatment plan. </li>
<li>Distribute medication in accordance with He-m1201 as applicable.</li>
<li>Report any safety, health or human rights concern immediately to the Supervisor. </li>
<li>Understand and ensure compliance with applicable state regulations. </li>
<li>Perform other duties and responsibilities as assigned. </li>
</ul>
 
Required Skills:
<ul>
<li>Ability to communicate thoughts and ideas well in both verbal and written formats in either group or one-to-one settings.</li>
<li>MOAB day 1.</li>

<li>Ability to meet required deadlines for documentation of care. </li>
<li>Ability to work independently, abiding by pre- made weekly schedule of individual, without direct supervision and demonstrate sound judgment and decision making skills.</li>

<li>Ability to obtain information about and to utilize community resources, activities and social opportunities relevant to supported individuals. </li>
<li>Ability to represent Community Choices and its services to the community with a positive and professional image.</li>
<li>Ability to work as a member of a team and carry out team decisions.</li>
<li>Ability to demonstrate computer competencies: utilize email and word processing specifically.</li>
<li>Ability to demonstrate a positive attitude under stressful situations with individuals, co-workers, and people external to the organization.</li>
</ul>
 
Work Environment and Physical Demands:
<ul>
<li>Work environment- The positions is typically indoors, unless the individual wants to go outside. The noise level is minimal at the residence. </li>
<li>Physical demands- Direct Support Providers must be physically able to lift up to 50 lbs. and to provide transfers as necessary. Walking and standing required frequently with occasional stair climbing. Must be able to push wheelchairs and operate wheelchair lifts/Hoyer and barrier free lifts. Emergency behavioral intervention must be followed in accordance with behavioral intervention techniques and individual behavioral plans.</li>
</ul>
 
Travel:
Must have access to personal vehicle during all scheduled work hours. Frequently travels with supported individuals utilizing personal vehicle as well Community Choices vehicles. Must possess and maintain a valid NH license and safe driver's record in accordance with Community Choices standards with adequate liability insurance.
