PCA Skills Trainer
The PCA Skills Trainer will meet with agency consumers involved in the agency's Personal Care Attendant (PCA) program. We are seeking applicants with experience in service coordination type work with persons with all types of disabilities and elders in the human service field, as well as those passionate about helping others and willingness to show a commitment to Independent Living philosophy. Applicants must have a strong ability to organize their time between home visits and required paperwork/documentation.
Essential Job Functions:

<li>Provide information, orientation and determine basic eligibility to persons with disabilities seeking Personal Care Assistant (PCA) Services through the Medicaid Personal Care Attendant Program</li>
<li>Complete an initial assessment with consumer in their/residential setting to determine their level of ability to manage their program.</li>
<li>Respond in a timely manner telephonically or in person to consumer inquiries and questions</li>
<li>Documenting consumer visits and contact notes in the program's database system</li>
<li>Assisting consumers in completing paperwork necessary to manage and hire their personal care attendants, as well as advising consumers how to handle various types of employer/employee dynamics and situations</li>
<li>Maintaining consumer case files and monitoring a consumer's ability to manage their personal care program on an ongoing basis</li>
<li>Troubleshoot for and with the consumer regarding issues with their individual program which could include one or more of the four core services; peer support, skills training, advocacy and information and referral.</li>
<li>Maintain working relationship between PCA Program and Fiscal Intermediary (FI) including, but not limited to; resolving member complaints and sharing information with FI as needed.</li>
