We are currently hiring for both full-time and part-time Associate Teacher positions at The Nest Family Center. No weekend work required.
Full-time shifts are typically 8:00 AM – 4:30 PM or 8:30 AM – 5:00 PM, Monday through Friday.
Part-time options include 8:00 AM – 1:00 PM or 12:30 PM – 5:30 PM, Monday through Friday. 
As a full- or part-time Associate Teacher, you'll arrive each day ready to support a stimulating and safe educational environment, in alignment with New Hampshire state regulations. Under the guidance of the Lead Teacher, you'll implement developmentally appropriate activities that spark curiosity and support growth in every child.
You'll engage meaningfully with children, foster positive peer interactions, and maintain open communication with families - keeping them informed about their child's progress and responding to questions with care. You'll also work closely with your fellow educators to ensure a cohesive, collaborative classroom experience.

GENERAL DESCRIPTION:
To provide transitional coaching to eligible patients in an effort to reduce the likelihood of an admission or readmission and create a smooth transition from hospital to home. 
 
 
Requirements:

<li>Participate in Coleman training and orientation sessions as assigned.</li>
<li>Working knowledge and ability of I&amp;R functions</li>
<li>Telephonically outreach patients and/or meet patients at the hospital depending on contract specifications. Provide each patient with a minimum of 1-in-home in-person visit within 7 days of discharge or referral. The intervention will focus on 4 key elements of the Care Transition Intervention (CTI) Model.</li>
<li>Conduct 2 to 3 additional follow-up phone calls to the patient within the 30-day post-emergency room visit or admission emphasizing the elements discussed in the previous in-home in-person visits.</li>
<li>Identify required Support Services and provide them to the patient as appropriate.</li>
<li>Identify unmet community needs and provide referrals and resources as appropriate.</li>
<li>Participate in weekly meetings/rounds will be held with clinical staff to discuss the status and progress of enrolled patients.</li>
<li>When appropriate, request via email a 60- or 90-day extension of patient’s participation in the TIC Program.</li>
<li>Coordinate activities with other ASAP agency staff, including but not limited to: I&amp;R Workers, Care Managers, Skills Trainers and Registered Nurses.</li>
<li>Document activities in progress notes, medical records, and Transition in Care database as appropriate.</li>
<li>Complete all documentation required for measuring quality and outcomes</li>
<li>Participate in activities to assist and promote Central MA MetroWest Transitions in Care in gaining visibility and trust with Community agencies, hospitals, providers and the public.</li>
<li>Meet monthly targets including enrollments.</li>
<li>Screen patients for eligibility.</li>
<li>Complete requirements for hospital orientation which include obtaining and providing proof of certain vaccinations, PPD on an annual basis, drug screening, and not smoking. </li>
<li>Agree to follow the HIPAA regulations for each contract partner.</li>
<li>Other duties as assigned.</li>

 A bachelor’s degree in human services, health, or a management-related field is preferred. A minimum of two years’ experience in a social service setting is required. To be qualified as a director, you shall meet or exceed the following qualifications:

<li>Be at least eighteen years of age, possess a BA, BS or Associates degree, have one of the following:</li>
<li>At least ten months direct work experience with school age children OR</li>
<li>Vermont On-the-Job Training certificate OR</li>

<li>Vermont Program Director Credential OR</li>
<li>Vermont Teacher Licensure OR</li>
<li>Master’s Degree in a youth-related field</li>

We're on the lookout for a dynamic Director to lead our After School Program for the 2025-2026 school year at Fletcher Elementary School in Cambridge, VT. As the Director, you'll be the driving force behind every aspect of the operation of the Healthy Kids Program Site.
 
 
JOB STATUS: Part-Time, Non-Exempt
SCHEDULE: 2:30 pm- 6:00 pm
PAY: $18.00 - $20.00 an hour
 
 
JOB CONSISTS OF:

<li>Guiding Compliance: Ensure that both staff and program adhere to state guidelines, Healthy Kids policies, and the rules set by our host school.</li>
<li>Bringing the Fun: Direct engaging hands-on activities based on the Healthy Kids Curriculum, covering fitness, STEAM, academics, arts, and more. Plus, don't forget about homework completion – you'll be the ultimate homework hero!</li>
<li>Managing Like a Boss: Take charge of essential management tasks, including maintaining accurate attendance and registration records, and ensuring proper staffing levels to keep the program running smoothly.</li>
<li>Keeping Everyone in the Loop: Maintain open lines of communication with parents and family members through the Playground App.</li>
<li>Team Collaboration: Communicate effectively and regularly with your direct report, ensuring they're up to speed on daily occurrences and addressing any issues that arise in a timely manner.
